Mark Greene, an only child, was raised by his mother, Ruth, and father, David. David Greene served in the United States Navy, and thus the family traveled a lot and moved around frequently. The most memorable time of his childhood was when his family was in Hawaii, a time he would later recreate toward the end of his life.
Mark entered medical school, married Jennifer (Jenn), with whom he had a daughter, Rachel. He completed his internship and residency in the Emergency Department of County General Hospital in Chicago, Illinois.

In the pilot episode, which takes place on St. Patrick's Day 1994, Dr. Greene is chief resident in the ER of County General.
Later he removes a hangnail from an older woman, who wanted him to remove it despite the fact she'd be charged $180.
During the first season, Dr. Greene's marriage becomes increasingly shaky. Jen begins an affair with a coworker, and the marriage soon ends, with Rachel and Jenn leaving Chicago.

Marks long term friendship with Susan Lewis develops throughout Seasons 2 and 3 until he eventually realizes that he loves her.
Mark makes a frantic dash to the station to tell Susan he loves her just as Susan is leaving. Susan tells Mark she already knows, but she leaves anyway.

Mark then goes through an emotionally unstable period with multiple brief relationships, including with Chuny (Nurse) in Season 3 and Cynthia (Desk Clerk), Season 4.
He eventually meets a British surgeon, Elizabeth Corday. After a period of dating (Season 5 and 6) they begin a more serious relationship and move in together.
Mark asks Elizabeth to marry him (Season 7). He then finds out that he has an inoperable brain tumor. He is operated on in New York City, and things appear normal.
Mark and Elizabeth get married in "April Showers" and one day later Elizabeth gave birth to a baby girl, they named Ella.
Season 8: The tumor returns, and Mark finds out that it is truly inoperable. He stays with Dr. Lewis (who has since returned from Phoenix) during his treatments, not willing to tell Elizabeth that his tumor has recurred. Eventually, he resigns himself to certain truths.
On his last day in the ER, he meets with the same older woman that viewers saw on the first episode of ER. She again has a hangnail, and complains about how painful it is. Mark tells her that he has an inoperable tumor, and that he has won.
He leaves the ER, stops his chemotherapy treatments, and takes Rachel on a last-minute trip to Hawaii to relive happier times. After several moves around the island, he suffers from increased symptoms. Joined by Elizabeth and Ella in Hawaii, he later dies peacefully at 6:04 in the morning. The last thoughts in his head at death is standing in the middle of the ER that is for once empty of people, along with thoughts of his family - some very dramatic scenes as his thoughts fade to nothingness.
Mark: [to John Carter] See, there's two kinds of doctors. The kind that gets rid of their feelings. And the kind that keeps them. If you're going to keep your feelings, you're going to get sick from time to time. That's just how it works.
Mark: I've been a jerk.
Carol: Worse.
Mark: Pain in the ass?
Carol: Keep going.
Mark: Total schmuck?
Carol: Bingo.
Mark: I was embraced by two men today. I find that highly unusual.
Susan: Some guys have all the luck.
Mark: [in a noisy helicopter] Hell of a life isn't it?
Paramedic: What?
Mark: Its a hell of a life.
[The paramedic points to his ear.]
Mark: I said, I want to have sex with your wife.
[The paramedic gives a thumbs up.]
Mark: I've given it a lot of thought, and I've decided to be spontaneous.
[Susan is moving to Arizona, and Mark is seeing her off]
Susan: [As her train begins to pull away] I love you!
Mark: What?
Susan: I love you... bye!
[Mark has just told Carter that he has an inoperable brain tumor]
John: Oh, Mark, I'm sorry. What are you gonna do?
Mark: Die, I guess.
Mark: Mrs. Raskin. It's been a long time.
Mrs. Raskin: The service isn't what it used to be.
Mark: What seems to be the matter?
Mrs. Raskin: I have this hangnail, and it's very painful.
Mark: I have a brain tumor, and it's inoperable.
Mrs. Raskin: What?
Mark: I win.
